A good handover should include the discharge summary, procedure or operative note when relevant, pathology and imaging results, medication list, warning signs, follow-up plan and a clear understanding of when the treating doctor wants the next review.
After you return home
IndiaMedHealth can help organise the flow of information between the patient, treating team in India and local healthcare professionals. This may include sharing new test results, arranging a video review, clarifying what follow-up investigation was requested and helping organise a second opinion if the clinical situation changes.
Local treatment where feasible
Some parts of ongoing care can often be delivered in the patient's home country. When appropriate, IndiaMedHealth can help the patient understand what information the local clinician needs and facilitate communication with the India-based treating team. The local clinician remains responsible for care delivered locally.
